To Request Services
Download the Request for Disability Accommodations packet (PDF). This packet contains a welcome letter describing services, FAQ and documentation guidelines, and the request for Disability Accommodations application. Once all forms have been completed and documentation gathered, make an appointment with the Student Services Specialist/Coordinator of Disability Services (573-840-9608 x3738) located on the lower level of the Westover Administration Building. Please plan ahead as some accommodations can take up to six weeks to process.

For New Students:

All new students are encouraged to contact the Office of Disability Services (ODS) early in the process, especially if you have questions about what documentation is required for eligibility purposes. Early planning helps to ensure accommodations are in place when classes begin. Academic accommodations are modifications to academic requirements or procedures that are necessary to ensure that qualified students with disabilities receive equal access to education. These modifications should not alter the academic integrity of the course.

Each person is unique in his/her needs, and therefore, services will depend on what each person needs in order to equally participate in the classroom setting. You will need to self-identify to the Office of Disability Services by completing an ODS Accommodation Request Form. The request form for disability services provides ODS staff with preliminary information regarding your disability and requested services. Written documentation which supports your disability will need to be provided in addition to the ODS Accommodation Request Form.

For Returning Students:

Once accepted as a qualified person with a disability, you will not need to reapply for services. Students are required to request the academic accommodations they would like to utilize each semester. To request current semester accommodations contact the Office of Disability Services prior to the beginning of the semester, and we will discuss your course schedule and needed accommodations. Students should periodically review their accommodations to make sure that they still meet their academic needs.

If you would like to discuss revisions to your current accommodations please contact the Office of Disability Services. You will need to provide additional supporting documentation if there is a change in your diagnosis.
